| Cancellation and Missed Appointments |

| Many patients are waiting for a dental appointment sooner than the one they already have and appreciate any opening which becomes available due to cancellations. |

| To accommodate these patients, a minimum of 48 hours or more advance notice is required for cancelled appointments. If less than 48 hours is given, a $75 payment will be required for the missed appointment prior to or at the time of your next appointment. Your dental insurance does not provide payment for missed appointments. |
